On Hit: 7 (1d12+1) piercing damage, and the target must make a DC 17 Constitution saving throw or take 7 (1d12+1) poison damage on a failed save (half damage on success). If the poison damage reduces the target to 0 hit points then they're stable, poisoned, and paralyzed for 1 hour, even if they regain hit points.
Tendril. Melee Weapon Attack: +7 to hit, one creature, reach 60 ft. On Hit: The target is restrained (escape DC 17). While restrained, the target has disadvantage on Strength checks and Strength saving throws. This creature can't use the same tendril on another target while it's restraining a creature.
Reel. Pull each grappled creature up to 25 feet.
Web (Recharge d6: 5–6). Ranged Weapon Attack: +7 to hit, one creature, range 30/60 ft. On Hit: The target is restrained (escape Strength DC 17). The webbing can be targeted and destroyed (AC 10; hp 5; vulnerability to fire damage; immunity to bludgeoning, poison, and psychic damage), freeing the target.
